Please print the first page, then reload your that page upside down to print the second page so that it comes out on one piece of paper with the top of both pages at the same end of the paper.

When you have completed the form, call Personnel Group at 213 485-8890 to arrange to bring the form in and to have your motorcycle inspected. Bring with you your
Valid LAPD Retired ID Card (see note below)

Valid vehicle registration card for your motorcycle
Valid insurance card for your motorcycle
Current Motion Picture/Television Filming Work Permit
To save yourself embarrassment, check the expiration dates on all of the above documents BEFORE you go in and be sure none of them have expired.
Suggestion: As a courtesy to the LAPD personnel who will be helping you, provide them with photocopies of these documents so they don't have to take their time to make copies. But be sure to have the originals with you in case they want to see the originals.
On the day you arrive,
enter the rear parking lot off Judge John Aliso Street between Temple and 1st Streets (east side of Parker Center).
Stop at the guard shack and tell the officer you are there to have your motorcycle inspected for your work permit renewal.
Use your cell phone or the phone in the guard shack to call the above number to advise them of your arrival and be guided by their instructions.
Park your motorcycle as directed by the office or the officer at the guard shack. You will probably be parking very near the walkways to the entry doors.
If directed to go to the office first, bring your form and proceed to Room 508
Please remember to wear you ID card while in the building.
NOTE: LAPD is in the process of issuing a completely new style of ID card to its employees. According to the retirement counselor's office, retirees will eventually receive these ID cards as well. However, the entire active cadre of personnel will be converted first. So don't expect to have to change out your ID card until about the summer of 2007.
